As rounds run low, agents naturally triage harder — accepting minor incompletes rather than resetting quorum over stylistic concerns.\n\n**Dead agent detection** — If an agent fails 2 consecutive turns (timeout, empty output, quota exhaustion), it's automatically removed from rotation.
